Suppressors surefire suppressor shot off

A Surefire 556-212 came off an m4 during live fire. I was not the shooter, so I cannot attest to the can being properly attached. The shooter swears it was correctly mounted, so I am wondering if anyone here has had a similar experience.

I should add that this was fast-attach ratchet mount type, affixed to a bridcage type surefire flash hider.

All but the 22lr model are fast attach QD models. I have several and have never seen one come off when attached properly. There is a reason though they come with that nylon brush, you have to swab them out every once in awhile to make sure you get good contact and lockup.

With their new lockup system I dont see how one could come off at all. Must be witches magic.

In 5 years I have never had one come off while installed properly. I know of plenty of people that didn't seat them all the way and it didn't lock on and was launched downrange. I work for the company by the way. Is it damaged at all?

I have run the same setup for extended periods of time under some fairly harsh conditions and never had any issue like the aforementioned. I have seen them get slightly loose on the locking mechanism, and I have seen them become difficult to remove once mounted.

I assumed that the incident was operator error, but I thought I would run it past the clamoring masses for an opinion.

No obvious damage to the can. Ears however are a different story.

It says in the manual it can be shot off in emergency situations where the can is stuck and needs to be removed. You just undo the cam ring and fire and it should shoot off. Im going to bet the can was not attached properly if this happened by "accident." The cam ring was either not fully engauged, the can not all the way on the mount or the cam ring came undone without the operator noticing.

Happens all the time. The spring clip gets annealed from overheating and the mount comes loose.

Usually all things are user error. Personally i have never seen a surefire come off accidentally.

Yeah, that was basically my take. I just wanted to make sure that there wasn't design flaw that may have contributed.

That is correct, this is why we avoided any kind of spring retention clip in mounting our silencers. I have personally seen five 556 cans shoot off guns and I know an operator who has shot another four off his issue gun.

We don't make a 556 can so I am not trying to promote my products over theirs by any means, just trying to explain the issue and help people understand why it is happening.
