He's cutting downward. So once the edge slices the skin, he can apply pressure toward the block to keep the grapes in place. That's why you can see a couple of them move at the beginning until they all get lined up.

It a one way valve, so it starts the syphon without needing to suck on the hose.
It's a check valve. The ball(s) can move in the body. When he pushes the hose down into the jar the ball moves away from the mouth of the fitting allowing liquid to pass into the tube. By rapidly pulling the tube up, the ball is forced by the liquid in the tube to seat against the mouth of the fitting keeping liquid from exiting the tube. Reverse and repeat until enough liquid primes the tube and a syphon is achieved. You've got to be reasonably quick to make it work. A buddy of mine has been using a brass one to get gas out of fixed jerry cans in the bed of his truck for years. The basic check valve has been in plumbing applications since man invented plumbing. Putting one on a tube like this is a little more recent.
