That's terrible.....that's why you conduct second safety checks after anyone has left the training area & come back. We all re-arm back up when heading out in town for lunch, but then re-do the checks before allowed back in training....

The only time a Accidental discharge occurs is when there is a mechanical issue with the firearm. I have had an AD when conducting malfunction drills with a 1911. The sear broke and the hammer followed the slide forward popping a round off.

But, I was following all safety rules and the round went safely downrange without damaging anything.

The example in the news article was obviously an ND. Live rounds were mixed in with Sims training. Live guns (as opposed to Sims barrels) were mixed in with Sims training. All four safety rules were violated (Never point your firearm at something that you are not willing to destroy; Finger off the trigger until ready to fire; Know your target and what is beyond it; Treat all firearms as if they are loaded).

The training team needs to be relieved (fired) and the training progam needs to be stood down until they get re-certifided by an independant authority.

This is ass-clownery at the highest level.
