Both. Our course started at 8000', climbed to around 10,000', dropped to around 9000' then climbed up over 12,000. Total distance was 12+ miles. My wife and sons saw one broken leg, a broken foot and a couple people with hypothermia. It was the most challenging physical thing I have done. It was also very rewarding and a ton of fun.

the one I ran involved swimming back and forth across a 75 ish yard pond at the end of November.
Physical challenge wise, not so tough, the toughest part was running 12 miles. Personal challenge wise, there were some tough parts.
