What would you guys say the cause of this is? It's a m&p 40 with a storm lake 9mm conversion barrel and I'm shooting 124 grain round nose bullets hand loaded with 3.9 grains of 700x at 1.135 oal. I get this maybe once every 75-100 rounds so it's not happening alot but it is frustrating. I put an apex failure resistant extractor in it and it didn't really change anything. Are my loads to mild or to hot or does that have nothing to do with this? Looking to get some feedback from you guys. Thanks in advance!

Ejection issue to me. Slide might not be moving back far enough to pop the case off the breach face. See if you can duplicate by hand. Obviously not an extraction issue since the case is quite extracted.
Makes sense, I'm thinking/hoping it's just my loads not being hot enough. Gonna throw a few tenths more powder in them and see what happens.
Just checked all my pistols. None would move the slide back far enough to feed the next round, without the ejector hitting the spent case. Check to make sure your ejector isn’t damaged
Everything I've read says the spring weight is the same for 9/357/40 which is 16 pounds.
I take it my ejector isn't supposed to be bent down....dang it! I wonder how that happened.
Not having an M&P, I can’t say whether that’s typical to the brand. But would guess not. Maybe an M&P guy will chime in.
Checked the web. Yep. Not supposed to be looking like that. Give S&W a call. I’ve had good experiences with them with my more “mature” Smith’s.
When putting the M & Ps back together, take care when the slide comes in contact with the ejector, if not lined up properly, it would be easy to force the slide unto the frame and bend the ejector. Here's photos of my M & P.
