One of our guns went off after a barricade when the operator lifted the bolt to unload the gun. Sent back to remington, they checked it out and returned it with about a 5lb trigger. The others were during various schools and training. I was told about the problem when I went thru my first sniper school in 95. The agency teaching it told us that standard policy for them to pull the cross hairs off target just prior to swithcing from safe to fire.
I can't testify to all the conditions of all the guns I've seen go off, but all were operational guns, used daily.

Are guns abused, yes. Triggers messed with, yes. Are guns neglected, yes. I'm just reporting what I saw and have been taught over the years.

Just had some guys at work ask me about this to day. I had never herd about the so called problem. I did tell them that it would not keep me from buying a third rem 700!
