It's only happened to me once. Believe me, from that point forward I made sure she was good and locked!
It's only happened to me once as well. As a kid getting to shoot one with a friend, he knew about it and didn't want my thumb getting bit. As an adult i was shown how to load it by another M1 owning friend and had just done so, when I went to release the bolt and as he was saying "Doooooont
!"...I slammed my thumb.
