That has happened in our warehouse several times.
Usually someone gets careless and knocks the upper rung off with the top of the pallet or the top of the lifting part of the equipment.

Always a fun time when you hear tons of metal raining down from 26ft up onto the concrete and wait till it all stops, then go try to stabilize everything so the employee can get out of the equipment safely.

Pisses me off every time as now I have to do all my work, plus deal with all this new mess. oh and lost product.

I remember the second week I had started on my first job that involved a hi-lo: A pallet filled with white vinegar in glass bottles that had been shrink wrapped in the most shittiest fashion had to be moved from the warehouse to the supermarket across the street.

And just my luck, I was the one who got to do the task  Good thing it was on ground level already. Three bottles fell off just as I leveled the fork out and went hand grenade against the concrete floor.

First hand experience of what chemical warfare is like.

eh, that's nothing. company i used to work for, someone hit a bridge because they didn't have the equipment low enough on the flatbed truck. then another guy dropped a forklift off a truck trying to transload it onto another truck.
