This shows to be a safe load in my rifle. Just pierced 1 primer. Never done it before and hasn't done it since
Why?
 
Could be any number of reasons. Bad primer out of the box, you made it bad putting it in, more powder than it shuold have been, dirt in the barrel, etc etc

My first guess would be you have a large firing pin or a sloppy firing pin to hole fit- what action ins this in?

How many before and since and why the wait to ask, what changed?
 
It's a R700 firing pin has been turned down and bushing job
Powder charge was not wrong
No dirt in barrel why you asked I have no idea
80 before 4 after
Probably a bad primer
 
Doesn’t have cratering on the other primers does it? CCI 450 are pretty tough primers
