Primer Troubles

Need some possible causes for this tiny hole next to the primer. It is in the primer not the case.

Brass is Winchester 7RemMag on 3rd firing. Load is 69.3gr H1000 under 162 Amax.

I worked up to 72gr of H1000 when I developed the load and had no pressure signs till then when I got some flow on primer indent.

Had this happen in 2 out of 20 shots today.. do I need to back off?

Primer pocket didn't seal the gas pressure, throw that piece away and any other that are loose

Primer pocket is tight, just has a tiny pinhole through the primer cup. When I've seen pierced primers before it was where the firing pin hit, this just seems in a odd place.

looks like it may be a primer failure
what kind of primers are they
