Redding FL die serie "S" issue

Hi

I have been reloading for a couple of years but still learning; recently had an issue with one of my dies.

Anyone ever had the problem when, on a Redding FL die serie "S" (.338LM cal), the expander gets loose and the decapping rod bents?
I stopped when it felt like something was out of alignment but it was too late. Should have noticed it earlier; was probably too busy thinking about the next range session; as a results, got a bunch of messed up Lapua cases on which the neck is now more oval than round, not sure if I can salvage these.
Redding CS was helpful and they are sending a free new decapping rod; nevertheless, on the same email where I was explaining the issue, I asked them on how to prevent this kind of incident but did not get an answer.
Would anyone know how to prevent this?

Do you have a bushing in? That should be able to get the neck back into speck.

Affirmative, .365 bushing titanium nitride. I hope it works, cases are 2 times fired Lapua brass; waiting for new rod to come in, then drum roll...
Still trying to figure out why this happened; my fault, or a "shit happen" situation? Would like to be able to stop this from happening again.
