It may have been a broadhead. The picture shows the arrow shaft but looks like the tip might have broken off. It also looks like there is a wire attached to it (?). WeirdLifelong bowhunter and shooter here. And I can attest that broadheads, especially the regular triple or dual edge ones, WILL put a serious case of DRT on anything they hit. I rather stare down the barrel of a pistol than have a fucking bow drawn on me with a gleaming broadhead on the end.......Or face a knife, unarmed for that matter. Same shit, and equally nasty.
Mechanical heads are nice and snazzy. BUT, if they do not open, they ain't worth beans. Montec 3-blades are my preferred selection for medium to large game.
I have heard of several bow related accidents over the years, usually because some blockhead did not take "sufficient backstop" into account and an arrow went through, traveled quite a ways and struck someone walking their dog on the next street. Good thing is that usually people do not practice with broadheads attached, so these accidents are non-fatal for the most part.

I agree. Something is off here. The article said the guy bled to death. The picture looks like he’s prepped for surgery. Not much blood present. Doesn’t look like a fatal wound. What is in season right now for archery?I noticed that wire (or string?), also. Pretty odd. But the exit wound, if the left side of the pic is indeed the exit wound, looks a little too clean to be a broadhead exit channel.
