What is making rounds do this?

Quick question as to why rounds are doing this out of an Accurate Mag. I’ve not modded the mag any, and do not know if the previous owner did or not. Any information is appreciated. Thanks.

Is this a new combination between mag and action or did this just start occuring? I had and am still having to a lesser degree the same issue in my Howa 1500 with the Accurate mag. I flared the feed lips slightly and this definitely helped but did not entirely eliminate the issue. My next test is to try a magazine with a binder plate in it in hopes the bullet doesn't snag on the feed ramp. Good luck.

This is a newly acquired rifle and now that you mentioned the binder plate, I thought both my mags had it in them. However, they do not. I just checked the rifle with the non binder plate mag and it didn’t cause the issue. I'm looking at removing the plate from the other one, but from what I can gather, I may be better off just buying another non-binder plate mag instead of messing with this one.
