When I would work triage in a busy ER, seeing over 100 walk-in patients in 12 hrs, there were many times it was better to write in the Triage note “Patient states” and then follow with a direct quote.
There were some doozies.

Gives the nurse and NP or Dr and good idea whats up before going into the room before walking in.

From vibrators up the keister to concerns of an STD due to seeing “goo” on his junk during sex to “this has been hurting for 8 months. Im going on a trip tomorrow and wanted to get it fixed before I go.”
 
When I was an insurance adjuster I worked a claim where the officer had the involved parties write out statements of their version of events. One of the classy ladies involved repeatedly referred to the other driver as “dat bitch” in her written report. 🤣
Had a chick make a right turn on a red light in front of me. 4 lane road. She wrote "I made a wild turn". Cop didn't cite either of us (he was retiring the next week). He let me take a picture of her written statement. That really helped as I was on the company truck.
