That's the exact reason why it is adjustable. I have the SR-556 and if it won't cycle on gas setting 2 then I open it up to 3. Some ammo will cycle on setting 1. The idea is not to use any more gas than is necessary to make the action cycle.
Your rifle doesn't have a tight throat, it has a short throat. What is happening is that the bullet is engaging the rifling upon chambering; which is causing the marks on the bullet and the three instances of the bullet being pulled from the case neck. I would check the COAL of your hand loads...
