    I have had this problem I just put on a crush washer on and no more problems some times it has to do with thread fit if its a loose thread fit it is more likely to work loose or if its a real short thread it will do the same sometimes

    It seams my Howa 1500 formal a .308 now a 6.5 47 Lapua does not wont to eject the 6.5 brass it will extract the round out but will not eject it out it just drops it when the bolt is pulled fully back. Now if its a live round it will fling it out just fine I know it has something to do with brass...
