blank load data sheet forms or PDF?

does anybody here have a pdf or blank form I could print out for working up my reloads? I searched but didnt come up with one here.

So I searched google and found a few crapy ones but not like I was wanting. I was wanting to have the load data and enough room to put a target either below or above the data and be able to copy and put them in a three ring binder.

I have an 8.5x11 Excel sheet with 10 of these labels on one page I can email you.

I have been using the generic label on this site. I would like to change a few things on it, but don't have the ability to modify pdf files.
