Signs of case head seperation, or loose primer pockets. I trash the batch when I see these or a few neck splits.

That's why it's important to keep records of what has been fired and how many times. I use an index card in the box and just put a hash mark in the right place to keep up with it.

Case head separation is a sign that your handloads have too much chamber clearance at the shoulder. Case head separation will never occur if you're handloads are made correctly.
Excessive chamber clearance causes your case to stretch, and that makes your brass becomes thin and brittle. Then the brass cracks when fired. Too many shooters don't take case head separation seriously enough. It can be dangerous.
