Gunsmithing Broken Firing Pin

I recently had a 6.5x47L built on a Remington 700, and the firing pin hole was bushed for the small rifle primers. I have about 300 rounds down the pipe, and went to confirm my zero at 100 this weekend only to find that the firing pin was broken off at the tip. The only thing that I have done since my last range session was to dry fire, and have my gunsmith re-bed the recoil lug. Any ideas as to what could cause this?

Maybe the internal contour of the bushing didn't match the contour of the reduced portion of the firing pin and created a stress point...or everything wasn't completely concentric causing the tip to be bent sideways when firing. Could be that the original hole was off center and the bushing was indicated off that off center hole.
