In the past, we also had people who were suffering from genuine mental illness as well as people with abnormal kinks and proclivities for things that would seem bizarre to most everyone else.
For the former, they received intensive inpatient treatment and were taken off the streets and housed in secure facilities if they were assessed to be dangers to themselves and public safety. For the latter, they knew to keep that shit in the bedroom and behind closed doors. If they tried to boast about it or showcase it in public, they were laughed at, mercilessly trolled and ridiculed, and ostracized by their communities. In the cases where their kinks posed potential harm to other people and children, they were locked away too. Every technological and engineering innovation we have in the world today, from rocketry to aviation to solid chip computing, were discovered and patented during the decades we took a proactive approach to mental abnormalties.
