A few days ago at the range for a local group from a county away. Everything went well, they were quick on the up take for never having tinkered with comms per-say before. Two of them had used employee issued gear with different freqs already programed in but nothing more than that. After that we started banging targets. This is where I fucked up, was so intent on the target per training, my brain was disengaged I guess. Had fired about 250 to 300rds then had a failure. The FNX has re-strike ability which is part of my immediate action drill, did that nothing racked the slide but did not look at the ejected round/case as I was so intent on not losing sight of the target. Next rd fired slide locked back, dropped mag a shoved in a full one but the slide failed to return to battery. Long short, squib was the stoppage but then the next one made that FNX a two'fer. Barrel much bigger now an was hitting. Found a another barrel should be here tomorrow.

Lesson had this been real, bad news. Glad I only shoot Winchester white box for play time. Ammo was the root issue, but operator could have prevented end result. First one for me sense I started shooting anything in 1955. Be-careful out there it all could have went much different,...

Good to hear that no one was injured.

Having a double fire has been a worry for me with training my wife and I for immediate action as muscle memory.

The lesson I take from this experience?
When doing real deal training only use, combat/self defense "Proven" ammo. White box ain't that! I knew that but, I was the one that got complacent so my fault 100%.
Now, how not to repeat same? Never do real threat training, with other than real threat ammo. Always glance at the extracted case with a FT fire in a semi auto, if it's a empty case, time for a battle field pick or you backup.

If your teaching someone to use a semi auto, instill in them what can happen an train them to glance at the extracted rd, you will only lose target sight for less than .25 to 1/2 of a second. Immediate action drills are required in some venues, an in all my years of shooting or any of the schools I've been to I don't recall ever being told to look at the extracted case. I've been trained to never lose sight of the target no matter what, even when changing cover. Does it matter as the weapon is now inoperable anyway? My take on a real two way not really, but on a square range yes
However where does that leave, train like you fight, an fight like you train?

Brings back some shitty memories. In the 1980's I was shooting IPSC pretty religiously. Had a passion for it. Back then there were no race guns...it had to look like an issue 1911 and had to be chambered for 45 ACP. I took several classes taught by Ray Chapman. What a great teacher. During my first class, I had a squib...I cleared the resulting stovepipe but did not realize what had happened. Ray did and yelled NOOOOOOOOOO but it was too late. I pulled the trigger.
Slide jammed
Grips blew into splinters
Magazine blew out the bottom.
Hand was numb for 24 hours...I remember thinking that I would never feel anything in my left hand again. Function was there but very little feeling at all.

Colt needed a new barrel and grips. Still have it...still shoots lights out.

I am overly paranoid today for obvious reasons

I took several classes taught by Ray Chapman. What a great teacher.
Met Ray a few days before the first SOF convention at his range. Great no B/S kind of guy. During the run up to the kick off there was a bet between to guys one of which had his left arm in a cast an sling. Any who, the bet was a concealed tire tool (57 chevy) was faster than a concealed 1911 the crowd gathered an the 57 chevy tool kicked ass each time. The one armed guy was throwing the tool an stuck the tool in the ctr of the target each time. Skinny fuck was extremely fast an to look at him very unassuming. Found out later who he played tag for, which explained it all.

Ray Chapman is one of those guys we all miss so very much. Lots of interesting an verifiable back round, once he opened up. The people an groups he trained is long. One thing about him if he was having a bad day, he could not be mistaken for hiding it,...LOL

He was a one of a kind individual. He had a no nonsense approach to teaching you how to use a pistol in a VERY practical way. The bet you described sounds similar to another bet I saw with my own eyes.

He bet anyone that he could stand 15 feet away from you and that on the "go" command, you would try to draw down on him before he could disarm you. I took 4 of his classes and that took place during three of them. If memory serves me right, I would say a total of 8 or nine people tried it but none succeeded including a couple or three LEO.

He was the first person I remember using the term muscle memory to teach us the importance of consistent and repetitive technique

Lots of interesting folks there for the convention. Finally but faces to como voices or otherwise. Lots of new(then) gear an a tactics class was always in session as were med ect. R K Brown an others had just returned from A-stan a passed some Intel as well. Interesting times back then,...

You started shooting in 1955?
Yes, started shooting at age 8. Grandfather taught me how to reload 22lr at the age of 10, which is when my father gave me the H&R Targeter he bought the week I was born. Still have it to this day. It's put more meat pound wise on the table than any other stick I've ever owned. From Quayle to Deer it's killed everything that was intended for the table, including fish.

Were these taken at a Soldier of Fortune Convention????? This looks familiar to me
Yes, right before the banquet was over.
Rual an a few close others were beside me but also at our table were the guys that worked the bomb that blew the hotel to shit in Vegas. The one with all the bolts numbered. Interesting tale about leading up to an right before the energy release. Somewhere on the net it is detailed as the reporter for Rolling Stone was in the mix with us when they were explaining what all happened. The convention even made news in Russia as one of the guys was wearing a tee shirt that said happiness is a confirmed kill. The Russian reporter took that as we were out to kill all Russians, hard to believe I know given events of those times? The Russian was not well received an had some sort of accident an left before it was completely over. The guy from Rolling Stone was cool, showed us all kinds of deleted photos from his earlier days of interviews an such. Have his photo some where as well as many others from those days.

You make the first one?

I was invited to shoot the first one plus Raul wanted to go, only reason I went. The next ones I passed on as other things were going on then. Kept in touch with Ray an others for years afterward,... hated it when Ray passed.

I sent you a PM. I don't think I was at the one pictured but I may have been. Went to two and went to one banquet and shot a three gun match as well but I can't remember if the match was at the same time as the convention.

I paid...not invited. You must have been quite the animal back then to get an invite to that crowd

An more yet, Top laying of the panel markers, Middle well you know, bottom folks moving to a Tactics/Med class. Yes from behind she looked great, but the front was outstanding. One small SA lady you did not want to fuck with,... even if you were not a FNG.

Man, I’m going to miss you old gents!
Don't bank it before you know the odds. My Great Great grandmother died at 107, she smoked, dipped, an chewed all her life, in the back hills of Ky. She died from a broken neck from falling off her horse. Many, many on both sides of my family, have lived past 100. I fully expect to partake of this cluster fuck that is about to happen, the main reason I spend 1.5 hrs at the Gym 5-6 days a week an bike in the house everyday. Nothing is free, to get something you have to give up something an I will trade work an pain, for payback.

I cycle 30-45 minutes a day 6 days a week. Not going 10 miles per hour either.

Push ups and sit-ups every other day. You’d be amazed at how much strength you can build just doing body weight exercises.

Also drink and enjoy good food.

I’m 57 and don’t take one blessed pill

Sounds like you might be part of the Howard Families.... long life is excellent.

Was shooting a USPSA match and witnessed a similar problem.
Wealthy lawyer dude, with STI , had a FTF and racked it to go back to town.
Saw the empty case flying to the ground and the shouting started.
Either the squib blocked the next round or dude heard me.
Must have been the first time by the way he acted.
Doubt he is a reloader and that is a good thing.

Supposedly the HK Mark 23 original design requirements called for it to be able to survive that without doing a kaboom.
I'm not sure if anyone has ever wound up "testing" that.
