Suppressors Form 1 sent back becouse of line 4h... help

Line 4h on a form 1 is so confuseing for me I need some help
I used (BZ trust,city,state,model,001) and it was sent back stating use full name
Should I use ( first name,last name, revocable living trust ,city,state ,model ,001)... Would that be right ore can i just use my full name, trust

A lot of people leave that line blank. I always put what I was going to engrave, XXXXX Trust, City, State. But every examiner is different

The link to the guide is for the ATF form 4. If you leave 4h on the ATF form 1 blank, then the ATF is going to think you do not intend to engrave the silencer you are making.

Engrave it with the full name of the trust, city, state, serial number. Abbreviate the state name.
