help finding cartridge documentation pdf/word doc

Can someone please help me out ? My Google fu is failing me again today.

I've seen numerous times on here a jpeg/pdf/word doc that had a drawing of what you need to document your loads. It had what looked like a visio drawing of a generic cartridge and lines to document case head to ogive, oal, powder, bullet, lot #'s etc. I just can't find it.

Can someone have pity on me and post it up?

Thanks for the fast response. That'll probably work. I saw a version that was a touch more simple.

SoftCock had something similar to what I'm looking for:

I don't need all the extra measurements. I just want something simple that I can put down the measurements for my loads.

I'm such a dumbass. It's in Doc's Reloading 201 post that is pinned at the top of the page! Damnit!
