In general they are a fixed amount and not something that is user adjustable but I do believe there are some high end mounts that are I've just never actually used one or had any need.
Can I say the 20MOA rail or the scope mount/rings is making the scope pointing 20MOA down (if the barrel direction consider is horizontal, the scope direction is pointing like a down hill) to give the scope elevation adjustment additional 20MOA up when shooting longer distance?
