Suppressors More form 4 questions

Sorry for the repeat...I did a search but could not find an answer to my specific questions.

1. When using a trust for a suppressor purchase is it necessary to send in fingerprint cards with the Form 4?

2. In which section and how do I sign the form 4 as the transferee....ie. sign as myself or sign as Blah Blah Trust?

no need for finger print cards. in section 15 just sign your name in the signature space. it worked for me 9 times!
