Seating die stem flaring at mouth creating problems!!

I have a Forster micrometer seating die. I’ve seated probably 1500 rounds or so on it. Lately I’ve had problems with seating consistency. I ordered a new updated stem from Forster. The problem I found today was that the .003 neck tension on my Alpha brass and even my .002 neck tension on my Prime brass was causing the mouth of the seating stem to bell or flare. This made the sleeve catch on the stem and problems with seating depth consistency were prevelant. It was also gouging the sleeve. It did this with the old stem and the new. The die is on its way back to forster for them to look at. I did a search and this sort of thing seems to be common. What are the fixes and experience of people here that may have run into this problem?
