At what height will trajectory be a concern ?

It's always an issue, it has to do with trjectory, the higher you are the longer you will range the target BUT that is not the true range. You only want the distance the bullet has to cover as the crow flies. Think of a right triangle where A is the vertical line B is the horizontal line and C is the diagonal line, C would be the path of the bullet but you would adjust your dope for the length of B since that's the actual distance lengthwise a bullet will travel.
 
If you're zeroed at 100', and that's 15' at 150 yards is about a 2%, so probably not even 1/4". It's dead.

It depends. What are you shooting? How accurate do you need to be? A good way to learn this concept is to archery hunt out of tree stands because you have to worry about trajectory on every shot. For 308, I think the rule of thumb is 15 degrees and 400 yds. Some people calculate it out on long shots. Others just aim a little low if they need to.
