I got this mount on a demo SB scope from Mile High Shooting, so I don't have the original box, but I will include one of my spare boxes from a 4602 that I have.
Included are the screws plus some extras that these mounts ship with, the wedge leveling device, and instructions.
Used a bit but in great shape.
