I saw your other reply with the bullet weight. 42.5 is .5 over the listed maximum for both Hodgdon and Sierra's reloading manual. I don't think that's the main cause of the failure though. I'm going to take a guess that the case is defective to fail like that. That's surprising for Lapua.

My thoughts are that it might be a defective case or a combination of a defective case and something to do with the sizing process that's might be over working the brass at the base.

You might take some measurements of a sized case at the web area and compare it to measurements of a fired case as the same place at the web. If there's more than .0015, then that might explain it and be wary of the other cases ending up the same.

Again, this is nothing more than my thoughts you might want to explore.
