Suppressors Stupid Question I know

I have a suppressor already but the business I bought it from took care of most of the paperwork and mailed it off for me.

This time I am buying a suppressor from an individual in Oklahoma. I already have my Form 4 ready (signed by Sheriff and Seller along with my fingerprint cards) What else do I need? Do I have to include a declaration of citizenship?

Finally, where do I send everything in to? (Address?)

Read the top right corner of the form 4, it tells you the address to send to.

You still have to send your Certification of US Citizenship form in.

Certification of compliance ATF F 5440.20 is the form?

Yes, send it in one envelope. Be sure to submit the forms in duplicate.

Not sure if that was just a typo but the form is 5330.20 found here.
