Suppressors trust question

Quick question about my trust. Should i include my address on the trust? I used nfaguntrust.com and there was no place to put it. My can dealer said he would put it on there. I searched the interweb and there is no definitive answer. I live in ohio if that matters.

I've helped write several (58) gun trust and mine do not have address of trustee. I've never included it and all have been approved.

my attorney included mine on my trust. different strokes for different folks i guess
