Engaged in their usual hand to hand combat, she pushed him away from her, and Sammy fell backward in a very awkward way, landing full force on his tail/butt.
He went down instantly and I knew it wasn't good, he was gone in ~2 minutes. My wife and kids are 110 devastated, he was an amazing dog, gave us so much unconditional love and affection, we can't find words to explain how deeply we're hurting.
