Most likely struck at an inward angle to "push" the metal in towards the threads and swell the last thread to tighten it up (It does work in some cases). When it didn't work the lock-tite came out...Shoulder dimples. That's a new one for me.

So what’s the solution? Chop and start over?
Hah you amatuers don't understand the brilliance here. Heat rises therefoe the top of the barrel will get hotter than the bottom causing uneven expansion and therefore deflection of the barrel due to heating up. Once this barrel gets to temp it will be straight as an arrow and a true tack driver.
Wow, it really bothers me to see people get screwed.
