If "she" didn't have notoriety, this wouldn't even be a headline. I don’t know if the Green Beret follow the same model as some other units, but if it happened in those place you are out for year before you can start over to get back in.

Standards are only good as long as we uphold them. Plenty of guys AD, stay in the unit "cause we need them". Plenty guys failed to setup weapons to function properly during weapons check, stayed on lead missions. Politics of it suck ass.

When carrying a pistol first became a thing I do I almost had an incident.
In the room where I gear down, pulled pistol, racked slide to clear chamber, intended to pull trigger to let hammer down.
Something in my brain screamed dont do it!.
I realized than I hadnt pulled the magazine....pistol was loaded and hot.
Pulled magazine and cleared gun the right way.
Knowing how close I came I hope to never get that close again.
No short cuts. No hurrying. No complacency.
So shit can happen, but it happens for a reason and its not mechanical error.
