I have a M9 that when I release the slide, the safet automatically goes from safe to fire and the hammer cocks to the rear. Anyone know how to fix this?

Is it a new production, or an older gun? And I just want to clarify, when you let the slide forward, it comes off safe, but the hammer stays cocked? Or it comes off safe and the hammer falls also?
There's a spring loaded ball and detent that keep the safety lever in the safe position. If it's a well used pistol the detent and/or spring could be worn just enough that the jar of the slide returning to battery pushes it over the edge and it cams forward.
