I wouldn't even go so far as to say that it WAS BP's fault. I can tell you for a fact that BP is one of the most safety conscious and strictest companies to work for. I'd bet a shiny penny that if BP knew the kind of decisions their company man was making, that they'd have run his ass off. Sure, he represents the company. Sure he was trying to save them money, but look how much their saving now?! If I had a nickel for every time I seen a company man trip over a dollar to pick up a dime, I'd be a rich mother fucker. Poor-boying a project, nine out of ten times, turns out more expensive in the long run. We all here like to say, " buy once cry once". Well, it's true for the oilfield as well.
What it boils down to, is probably some ol boy's bonus. And it all started with, "uh...boss?..yeah, uh, I fucked up and pulled the pipe trough the rams..and now we seeing rubber in the returns...oh, and we lost one of the pods...so...Situational awareness would dictate that this may come back into play at some point."
"Never mind that, we don't have time for it."
But no, that's not the end of it. They say, at least I'm always told, anyone present has the power to shut down an operation any time someone sees something that is unsafe to continue. Now I realize that's easier said than done for the lower totems on the pole, especially after heads are knocking over other issues. But still, anyone who was dumb enough to go along with those plans was a non thinking unit.

We had a particular company man once who was dumber than a box of rocks, had no practical experience from what we could tell, and seemed to BS his way through his work. We got on a really sour well that flowed maybe 3% H2S. We wanted to flare the gas on the backside. He wanted us to mask up and work under SCBA. It wasn't until some cows moseyed up a draw below that we warned him about the price of dead beef and all that. Well then it was OK to flare it. Seems he didn't give a shit about us, but the cost of some dead cows was a real concern. We weren't polite to him, and no harm came to us.
