Rifle Scopes Loose Reticle?

I have a Leupold MKIII 6.5-20x50 with the target turrets. Last time I went shooting was the first time with the new scope. I shot it and was getting 3 shots under an inch and shots 4 and 5 would be 2 inches off. I know fliers happen, but one shot would go 2 inches left and the other would go 2 inches right. Then tonight I was looking through the scope and noticed the duplex canted significantly left. Is it possible the reticle shot loose(hence the fliers) or did I just mount it crooked?

It's probably loose. I had the same problem happen recently with another scope, sent it back to the manufacturer for evaluation, and was later told the glue that held the reticle failed. And the reticle appeared to be cocked when I looked at it after the scope had been carefully leveled. The main indicator was that my shot groups were consistenly about 1" high and 4" wide with no wind; when I mounted another identical scope, the groups were round again.

How did you mount it? What did you use to level it? It's possible it's loose, but I've seen far more than have just not been mounted straight.

I'm actually not the one who mounted it, my gunsmith did it and he's very reputable and reliable. No one is perfect, but he strives to be as close to it as he can so I'd highly doubt its crooked...but I also hate to think about having the scope be broken.
