New M-BRACE Scope Mount $279 from American Rifle Company

Mine were brand new ordered last month. I haven't had issues in the past, so I didn't even think to check the screws.

Yeah that’s strange, they probably didn’t check the back of the screw head as it dropped in and screwed in normally.

If you reach out to them I’m sure they will take care of you.

Bad batch of screws for sure, those shouldn't have left the screw manufacturer with that underhead defect.

Believe it or not, low quality counterfeit hardware infiltrating the supply chain is an issue; not saying that's the case here, but it's a possibility. From time to time counterfeit hardware even makes it into the aerospace supply chain, which gets ugly... especially if those counterfeit bolts and their falsified certifications are found after they're already installed in a nearly fully integrated spacecraft.
