The form that includes instructions for sending one in and pricing is located in the Library section of their web site. I would have never found it if they hadn't told me.
Reticle change:
First Focal Plane scopes costs $400.
Second focal Plane scopes are $250.
They charge, I think, $16.00 for shipping and handling on top of that.
There are some limitations on reticle changes and they are explained on the form.

I would think it would be cheaper to just buy a new one with the reticle you want than to buy a used one and change it.

Anybody know if they're up and running yet on the Velocity reticle changes for the 3.5-15x's? Sent an e-mail yesterday but no response yet.

Not always. I have sent several back to convert to Mil/HS and I feel like it has been a good deal, especially on some scopes I bought a while back.
