Actually the 5.45 S/W you referred to that was keyholing turned out to be a 556 bore chambered for 545, not bad ammo. The owner did remove the FH and inserted a 545 round nose first in the muzzle and it slid in to the case, tried a 556 round and it didn't go past the ogive.
In this case I still want to hear fif there's a muzzle attachment or can.
