What is this mount and how do I fix my problem

I bought this mount a while back and am finally getting around to finishing my build. I can't remember who makes it. If anyone knows please let me know.

I mounted a scope and the rifle was hitting so low that after I cranked all the elevation I could into the scope it was still hitting 12" low at 25 yards. The scope was a Super Sniper. I wondered if it was the scope or rings so I put a different scope and rings on it. I got the same result.

How do I fix this problem?

That looks like a DIP mount.

The name should be on it somewhere. Is it canted? What rings are you using?
12 inches low at 25? the rifle would basically be pointed into the ground and I would think you could see this before you ever even got behind it.

Did you bore sight it? I think you have gone the wrong way with the elevation turret. The arrow that points to the word "UP" is to move the impact, not the cross hairs.
