Which ATF Form?

Which form do I use to apply for a stamp and register a rifle as an SBR? Is it the form 3?

I was thinking it was 1 but I'm sure someone here who lives in a state that allows SBR's will confirm.

It's a Form 1. Form 3s are what FFL/SOTs and T2 manufacturers use to transfer NFA items. Form 4s are for when a common joe blow off the street BUYS a NFA item.
