So the episodes play out like this:

Like most other ER docs, my shifts in the last few weeks have had a fair amount of Covid patients. Yesterday and today were no exception to that. What did stand out was that both days I had a patient come back to the ER that I had seen several days ago with Covid and sent home at the time. The decision on whether to keep a patient in the hospital with Covid or send them home can have multiple variables involved, but, in general most of the time it comes down to one variable- their oxygenation status. If they are above 90% on room air they go home, if they are below, they stay.

Both of these guys when I first saw them (on different days, they were not together) were in the mid 90s on their oxygen saturation. Both of these guys were in their mid to late 40s and one had hypertension and the other had no medical history other than being about 25 lbs overweight. I distinctly remember both of these guys because they were loud and obnoxious about not being vaccinated and loudly proclaimed how proud they were about that decision and each one stated to the effect that they are so tough they won't get really sick and they'd rather die than get vaccinated. Also, as the story always goes, the only reason they came to the ER is because a family member or friend had forced them to come and get checked out. For some reason it seems really important to them that they repeatedly state that.

They each seemed a little surprised when I stopped their vaccination proclamation mid sentence and told them verbatim, "I don't give a shit whether you are vaccinated or not, it does not affect me or my family." I then tried to clue them in that they are the only ones rambling on about vaccination stuff. I tell them what they need to know and that they are OK to go home to which, of course, they revert back to saying they were "forced" to come to the ER.


Fast forward about 4-5 days and that brings us up to yesterday and today. Dude #1 came back yesterday brought in by ambulance at the very beginning of my shift. He was in moderate respiratory distress and his oxygen saturation when the medics picked him up was about 78-80% on room air. In the ER he was on high flow oxygen via nasal cannula with an oxygen sat of 90%. He was still working moderately hard to breathe at that time. I told him I needed to know what his wishes were with regard to being intubated and put on a ventilator if his O2 sats continued to drop. To this he burst out in tears and is literally yelling/crying with just about the same decibel level as his earlier obnoxious proclamations 4 or 5 days ago. He also asks if it's too late to get the vaccination. Ummm yep, it is. He of course states he wants "absolutely everything done", despite his tough guy statement to the contrary a mere few days ago. He also asks if he can be transferred to, "Stanford or somewhere like that." Ummm, nope.

I admit him to the hospitalist and he gets a bed about an hour later upstairs in the step down unit. A couple hours later I get an update from the hospitalist that the guy has gotten worse and is now on 100% oxygen face mask and is sedated and prone in the bed. Hours later, less than 30 minutes before the end of my shift, I hear an overhead code being called in the step down unit and of course it is this guy. Unsurprisingly, the guy does not make it through the code and had no response to code resuscitation. On my way back down to the ER, I had this sudden realization that I had not eaten all day and I was starving. I diverted to the cafeteria and ate a big sammich which was surprisingly good.

Today, same scenario plays out with dude #2 who comes back in respiratory distress but not by ambulance. I won't bore you with all the details but his loud crying and his clinical course was very similar although by the time I had left the hospital this afternoon around 4pm, he had not coded yet but he was sedated and intubated. Just prior to writing all this about 30 minutes ago, I got a text from the hospitalist that the dude was dead.


Disclaimers:

1. I don't give a fuck if you are vaccinated or not. In my friends group/mountain bike group/shooting group, we have people who are and are not vaccinated. The only time this comes up is when we are jokingly fucking with each other and making fun of people who grandstand about their status.

2. With regard to vaccination, probably the only thing I really do give a fuck about is making sure people have a choice and can make that decision for themselves. That being said, you need to accept and live with the consequences of your choices and not be fucking loudmouths before the consequences or during like the two little bitches above.

3. I have been an ER doc for over 20 years, I don't give a fuck what you think you know about medical issues.

I must admit to being perplexed. If you are indeed who and what you claim you are (I have no/claim no medical credentials to accurately make a determination, so I will take you at your word), then why come here and dump what appears to be a lot of vitriol? Particularly in light of your comment number three?

I am unclear as to the point(s) that you are trying to make. Also, you seem to be slamming two people that didn’t get the jab. In your opinion, would the jab (well in advance) have improved either/both of their chances of survival? Did you/would you have prescribed Hydroxichloroquine ? If yes, why ? If no, why not ? What did you prescribe to them and was it simply a matter of ”too little, too late” ?

Well then, the gene pool is a better place if they were that stupid. If they were really tough they would have gone to the feed store and got some horse wormer.

Dune, the numbers are really good for the right patients who get Regeneron. I feel like it is almost a luxury to have it available.

I'm extremely glad our medical and frontline people are there when we need them. At the same time, I've been fortunate enough to hardly need them in *my life.
This story could have been about how they both were obnoxious and ate rotted turkey out of the cafeteria at the hospital and both died. 99.6% of people who ate the same sandwich didn't. But those 2 did because they didn't have the avocado to counteract in a freak turn of events. Will I get the poke? Likely not but you never know. Will I die,? Of course, and so will everyone else. When is the only question- to which none of us knows. I remember when aids first made the news and everyone was worried we would all contract aids in a public restroom and wipe out all humanity. This too shall pass and be but a blip of memory. To those who lost loved ones, it will be a time of reflection, and always be a void individually.
What will really leave a mark as a people are the freedoms we've lost along the way. and continue to lose as we live in fear and pine for safety at its expense.
To each their own. Poke or not I could care less, but standing up for what is dwindling while we fight over the hysteria, i couldn't possibly care more.

So, my take as a once-very-long-ago-has-been where the OP was, sometimes its worthwhile to relay a little PSA to get something off the chest, de-briefing if you will, that might serve a dual purpose. COVID deaths are still happening, more likely when risk factors present, and not to blow off symptoms. Vaccinated and un-vaccinated. The irony of the being on the "no-vaccination" soapbox to being in a pine box within a week ...well take it for what its worth. We normally relate it to what they like the "jabbers" would say, but sounds like both individuals had risk factors. Both should have been smart enough to accept an Rx. Home pulse ox's do a ton of good too. I might hold back on calling deceased bitches.

I haven't seen a single post by the er doc about the people that died in car crashes that didn't wear a seatbelt. Or the ones that did wear their seatbelt and still died. Or a single story of the 250,000 that doctors kill each and every year.

Maybe this OP was to relieve the guilty feelings for not sending them home with a script for a dewormer or a malaria pill.
It's funny that trillions have been spent and we have these jabs but nothing for the early stages. Come back when you are about dead and we'll sedate you till you die?

I passed the Rona on to my wife back in early March. She had it much worse than I did. I took her to the ER and they kept her for about six hours administering an antibiotic that I can't recall. Said it was the same thing Pres Trump received. She felt better, but deteriorated over the next week. Back to the ER for six days on oxygen and antibiotics. Took her a month or more to get back to normal. The nurses told her the first treatment most likely kept her off the vent.

It seems treatment varies in different areas.
 
some people think this guy is a clown on how he comes off, i get it. but in the medical field, death is second nature. other than kids, death is just another day on the job. it's unfortunate. prayers for their families.

Doc, being on the front line and dealing with this everyday, what do you do/take to avoid or decrease your chance(s) on catching this? is it just a simple mask? i would safely assume you're vaccinated but is that enough?

Sounds like the doc has an ego problem...........as many of them do. I'll bet you think masking up is a good thing too.

You're supposed to be a professional, regardless of the bravado of some loudmouth's in your e.r. So act like it and put your Jesus complex back in your pants. You really seem to take joy in the misfortune that these two have suffered. That makes you an asshole if you get your kicks that way.

You can't blame people for questioning the vac's and doctors. All of the bad factors that are coming out, such as people being re-infected after taking the jab, and now discussions of needing a 3rd jab, political angles and agendas. Hell, look at all of the infighting going on in your own medical/scientific community, yet fauci is the prima donna. The all knowing one...............You'd be a good fit with him on the golf course.

Some of you physicians really give mirrors a workout everyday. My only hope is that I don't end up with a fellow like you as the attending e.r. physician.

Doc, that is a darn good story. I'm not getting the shot but not bragging about it either.

Now I will say that I'm a pretty tough guy except for four things. I can break down pretty easily over them. Not necessarily in this order.

1) Needles
2) Dentists
3) Enemas
4) Urinary catheters

what about the human form of ivermectin? Was that offered? If not, Why not? Seems like some therapeutic is better than no therapeutic.

We're all human and I'm sure we've all made mistakes. i know i wouldn't want the responsibility that most Doctors have on a daily basis when it comes to life or death. i don't know rat shit from rice krispies when it comes to anything medical so i don't ever tip toe into that subject to purposely make an ass out of myself. this guy is an Er Doc, not your family annual heath check up doctor that gets to know your whole family and dog. his compassion isn't going to paint the picture that most people here believe how Doctors should be. any human life lost is an unfortunate deal. he mentioned he's been an ER doc for over 20 years... in my opinion, that's probably long enough to make someone deal with death a little different than the average person would deal with it. He's probably seen more dead people in a week than most on here will see in a lifetime. Both also referred for Regeneron, as we finally have that available in the county as of 2 weeks ago. Neither one pursued it.

I can understand not getting the vaccine, but not to take ivermectin, HCQ, D3. Zinc or other dr suggested treatments...well,

1. Its impossible to prove a negative. In the above situation posted by the op theres no way of knowing if a different action could have changed the outcome.

2. Ive herd of similar situations happening with alcoholics that have destroyed there liver, drug addicts whose body is destroyed or obese people on there deathbed. Some can live with there choices some cannot.

3. The original post feels like a sermon I could have sat through on Sunday. Exchange life for soul and vaccine for God and it reads the same. When did doctors become preachers?

Meh, this isn't a professional venue, he’s just venting to the boys. Im sure you vent about your job, and that's likely not the face you put on at work.

Prove he did a shitty job of treating the patients and i’ll agree he was unprofessional.

yeah, it sounds weird but a 23 yo girl who used to work for me got Covid in December, the same time a 75 year old woman former alcoholic/smoker, and 2 time cancer survivor got it and a 62 yo man with Parkinsons got it. Few symptoms for the Parkinsons sufferer, he had a bad cough but felt fine. Hit the cancer survivor hard for a couple of days but she sought out the treatment Trump got and it cured her quick.

The 23 yo girl, in perfect health, it rocked her world badly. she was feeling terrible for 8-10 days. she was in contact with a doctor from day one. She told me she didnt want to take some of the meds they were trying to give her (ivermectin) because it sounded scary and she didnt want to do it. i told her "you should follow the doctors direction" and when she did, she got better immediately.

The moral of the story, people deny meds even against their best interest sometimes. why? who knows.

So, my take as a once-very-long-ago-has-been where the OP was, sometimes its worthwhile to relay a little PSA to get something off the chest, de-briefing if you will, that might serve a dual purpose. COVID deaths are still happening, more likely when risk factors present, and not to blow off symptoms. Vaccinated and un-vaccinated. The irony of the being on the "no-vaccination" soapbox to being in a pine box within a week ...well take it for what its worth. We normally relate it to what they like the "jabbers" would say, but sounds like both individuals had risk factors. Both should have been smart enough to accept an Rx. Home pulse ox's do a ton of good too. I might hold back on calling deceased bitches.
