Gunsmithing Broken Surgeon M-16 style extractor

I Just had a Extractor break in a brand new surgeon action (6) rounds fired....using all 6 rounds of lapua and loading a Prime ammo round...Surgeon stated that would send a new one, but ask for payment info, I was really not happy with that, but it spawned two other questions:

1) the break was a straight clean shear, honestly looks like a material defect, but what if anything else could cause that?
2) are M16 style extractors from other manufactures compatible? i.e. badger

I called about a year ago for a replacement firing pin that was my fault it broke. They wouldnt accept payment and had a new one to me within a week.

Did you have any resistance when closing the bolt? At the point where the extractor clicks over the rim on rotation of the bolt.
The reason I ask is if there isn't enough clearance on the OD of the bolt nose the extractor gets slammed in the most rear facing portion of the barrel. Ask me how many times I've done that and seen that?

yeah, that is almost exactly what happened, it felt like the round was seated too far out but the bolt jammed, I tapped it a bit but not hard, and put it away, got home wiggled it and the extractor fell tip fell out
