help -- very strange ejector roll pin issue

Hello, I started getting consistent FTE's with my AR-platform 6.5 Grendel recently. Looked at the ejector, noticed it appeared "permanently" pushed about halfway in, and didn't have much spring force behind it. When I took out the ejector (first one I've removed), I notice a half-moon channel punched through it. Thinking there's no way that could be "factory", I'm assuming that the notch in the ejector just slammed through it. Has anyone else ever seen this? Any idea about the cause? The rifle probably has 250 rounds (total) through it. Here are close-ups of the pin:

Wow! Crazy looking! Is the gun still cycling correctly? Is the ammo still ejecting in the same location? It absolutely looks like the ejector punched right through the pin.

No, I was getting consistent failures to eject, every shot. The case ends up being turned about 40 degrees, but then jams with only part of the case sticking out of the ejection port.

I just want to know if there's something I was doing wrong, or if there is such a thing as a poorly-made roll pin (like some of the MIM parts you hear about.) The ammo was loaded pretty hot, judging from the flattened primers, but as anyone who reloads 6.5G knows, the range for powder charges in the available data are sometimes very narrow (and I was not at the high end).
