I've got a couple of Surgeon 591 actions coming and I'm wondering if my old Holland M700 wrench is going to be compatible.

No it will not work on a surgeon, The second, left anti-bind rail on a surgeon will not allow it to work.
Can it be modified to work?
I think you could slit a anti bind rail slot into the wrench, just make sure the wrench torques on the race way body and not the anti bind rails. If you bend or distort the anti-bind rails you could end up with bind rails.
Yes it can. I have the same wrench and when I tried my first Surgeon years back, I noticed the problem. I set it up in the mill and cut a slot in the wrench where it needed it. Works perfect.
I have that wrench and it works in the Surgeons....once someone modified it with a mill for me. 
I know there is a rail in the left side of the raceway, is there also one on the right?
