NASA’s Mars Climate Orbiter, which was lost on September 23, 1999.

The failure happened because one engineering team (Lockheed Martin) used imperial (pound-force seconds) for thruster impulse data, while NASA’s navigation team assumed the numbers were in metric (newton-seconds).

As a result, the spacecraft entered Mars’ atmosphere at too low an altitude (about 57 km instead of the intended 140–150 km), causing it to either burn up or skip off into space.

Ukraine just had some big wins against a ship and a few oil facilities deep in Russia. He's probably pissed.
He sank a Uke ship last week, so the tit for tat game continues. There’s an argument to be made that he is intentionally draining NATO resources. That could go very bad for either Russia or NATO, depending on how the ball bounces.
