However, given that specific unit, their mission, and the rapidity with which the "maintain visual separation" request was made following the initial traffic call, I suspect there was strong institutional/operational culture (techcedure) there.

I'd also say since the evaluator was PM and was the one who asked to maintain visual separation from traffic....failure to do so is ultimately on HIM.

ATC: "call the airport/traffic in sight"

PM looks over at PF for a thumbs up or something that they see what was asked.... or asks PF "do you have the airport/traffic in sight"... before responding.

I have had PM's make calls on airports or traffic that I, as the PF, hadnt/didnt see. We had a conversation about that after the fact to make sure the person flying the airplane actually saw what was needed before making that call. I know I wont call anything without the PF saying it or giving me a thumbs up when I look over.

NOW obviously there are cases where say the airport is off one side or the other and the other guy physically cant see it, but generally there is a quick conversation about that and the person who see's it calls it in sight.

I don't know if it's something that is really happening or not. I'll just say that there's a lot of videos of people "accidentally" discharging their weapons to say that it's the fault of the gun 100%. Add in the factor that a majority of accidents happen due to being comfortable in routine situations and safety is no longer priority. That happens in every job from mechanics, electricians, fishing, logging on down.
To blame it on someone else is common in today's society that lacks personal responsibility. The largest section of any phonebook was attorneys. Dewey Sue and Howe.

What’s makes me nervous is the amount of possibilities- Sig says it’s bad boosters, bad ammo , poorly trained stupid Americans ,it’s aftermarket triggers or other aftermarket parts.
Others claim it is a flawed design in the trigger sear interface of poorly designed over complicated striker and or sear .

Shit at my airline a day 1 new hire in the airplane can take the airplane from a 30+ year Captain if they deem it necessary for safety of flight.
That's exactly my point, said more directly.

With modern CRM, a crewmember in either seat can take the aircraft if safety is endangered. Insinuating an evaluator is incapble of doing so simply because the evaluee may have signed for the aircraft just isn't accurate.
